We are conveniently located approximately 7 blocks south of Sarasota Memorial Hospital, at the corner of Boyce and South Tamiami Trail.
Our regular office hours are Monday through Friday, 8 am – 5 pm.
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Dr. John T. Moor at Advanced Sports Medicine Center, and they will contact us.
If you have an urgent medical need and need to talk to our providers after our regular hours, please call our office at (941) 957-1500. A doctor is on-call 24 hours a day. Your call will be returned promptly.
Should you need to cancel or reschedule your appointment, please call our office at least 24 hours before your scheduled appointment.

Please contact your insurance company or you may call our office at (941) 957-1500 to find out if we accept your plan. We will always work hard to adhere to the requirements of your insurance plan. To help you maximize your benefits, please bring your current insurance card to every visit.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.
Please keep in mind that health insurance plans’ policies, procedures, provisions, and restrictions are constantly changing. We recommend you call your insurance company for your most up-to-date plan coverage.
If you have a question related to your bill or insurance, please contact our accounting department at (941) 957-1500.
Office Visit: If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.
Physical Therapy: You will need a referral from your physician or medical provider (primary care doctor, nurse practitioner, orthopedic surgeon, podiatrist, chiropractor, etc.) for insurance to pay for your physical therapy treatment.
Advanced Sports Medicine Center will accept an initial referral by fax, by phone, or by email from your provider, and we will obtain the needed signed prescription within days of your referral, or you may bring the signed referral form with you to our office.

Prescriptions and refills for established patients of Advanced Sports Medicine Center are routinely issued during our regular office hours (see above). You can make your request by calling our office at (941) 957-1500. All refill requests are reviewed by a physician. If your prescription refill cannot be approved, one of our office staff will promptly contact you.
Refills are generally not issued during the evenings, weekends, or holidays, as the doctor may not have access to all your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
If you need a copy of your medical records to be faxed or sent to another physician, please give us at least 48 hours’ notice. If you need X-rays or MRI films released to another party, we are more than happy to comply. Please realize that all medical records are the property of Advanced Sports Medicine Center and therefore, depending upon the request and party involved, a small fee may be incurred. Rest assured that your medical records are kept completely confidential. This information will only be released upon your written request and approval.
