Once you have made your initial appointment, you may preregister by downloading, printing, and completing the forms below. Doing so prior to your visit will help speed up the new patient registration process.
Please be sure to bring forms with you to your first visit along with any diagnostic films or other test results that may have been ordered or performed by another physician.

What to Expect
When visiting Advanced Sports Medicine Center, we recommend that you arrive 15 minutes early for your appointment so that you can complete the required paperwork and allow us to make any necessary phone calls regarding insurance verification or pre-certification.
If your insurance company requires pre-authorization or a referral from your primary care physician, it is in your best interest to obtain this information prior to your scheduled appointment so as not to delay your treatment.
If you are a new patient, we encourage you to download and complete our new patient forms above, which will save you time in the office.
Although it is our goal for patients to have no wait or a minimum wait at most, we sincerely appreciate your understanding and patience during emergency situations. Please know that if you have an emergency, we will extend the same privilege to you.
